We take the time to create a personalized treatment for each patient and we offer a wide range of services, from eye exams for adults and children, medical eye care, contact lens exams and fittings to a full eyeglasses boutique. It is our goal to make your visit with us an extremely pleasant one and to send you home with great vision.

Patient Forms

You can save time and expedite your first visit by downloading and completing the forms in our “New Patient Package” at home. Simply hand the forms to our receptionist at check-in. Also, remember to bring your insurance card and an ID with you to your first appointment.

What to Expect at Your First Visit

Your first visit to our office is designed to get you better acquainted with all we offer as well as introduce you to our caring staff. On your first visit, we will review your medical history and evaluate your eye health. We will take retinal scans and explain to you whether or not you will require any treatment.
